Schedule Change Request Form

 Schedule Changes WILL be made for incorrect placement of core classes.  Schedule changes WILL NOT be made for requests such as lunch or elective adjustments unless it it critical to student success.  Please drop off your completed request form in the front office or at the counseling desk. 

Please be patient with your request, your counselor will review it in the order it arrives.  Counselors will assess each request and make necessary adjustments.  Please note: not all schedule requests may be accommodated.
